The following forms are the health forms that shall be completed on each student attending any one of the preschool or early childhood classrooms associated with the Portage Community School District, Portage Partnering Preschool Program. Please take a moment to complete these forms. Return the completed school district form to your child's preschool program or directly to the the District Nurse via the Gerstenkorn Administration Building at 305 E. Slifer Street, Portage. If you need to speak with the district nurse regarding health concerns for your child, please contact her at 742-4879, extension 4022.
Students enrolled in any one of the preschool programs, shall provide written a written immunization record to the Portage Community School District. Immunization compliance is tracked by both the school district under the Wisconsin Student Immunization Law and the preschool under their licensing agency through the State of Wisconsin. We realize that this may seem duplicative, but we are trying to work together to provide the safest environment for your child.
If a student requires access to an emergency medication or insulin while at preschool, complete one form per medication or procedure. The medication must be in the original pharmacy labeled container, not expired, and presented to an adult at the school with a completed medication form. The school district nurse will provide Portage Partnering Preschool staff the required training per statute before the medication may be administered during preschool hours.
